Remote Patient Monitoring is the process of using technology to record patient health data remotely for review by a provider in a different location.

The 100Plus portal hosts all the information needed to monitor your patient's health. In the portal you can track patient enrollment, check if patients have provided necessary information to obtain an RPM device, accepted User Agreement, and track device shipment. Once RPM devices are being used, you can also keep a tab on their clinical data through the portal.

Medicare reimbursement is given to medical providers for prescribing and reviewing data produced by RPM devices that capture, save, and transmit patient's health data. Each month a medical provider can receive up to $200 per patient for CPT 99454, 99457, and 99458 (x2). Additionally, medical providers receive a one time reimbursement for initial setup and patient education of up to $24 per patient.
